Registration Forms

To become a DREAM mentor or mentee, it is necessary to register appropriately with the organization.  Below you will find all of the forms required to join the DREAM Program.  Upon completion of the forms, please return them to a member of your Local Program or mail them to the DREAM office closest to your residence.  Click here for the mailing address.

New Participant Surveys

In addition to the forms we require, we ask that each new mentor or mentee fill out a survey when they enter the program so we can track their experience and the success of the program.  This survey is ideally filled out when you submit your forms, but can be submitted within two months of being accepted into the program



  • New Mentors
    • New Mentor Form
    • In addition to your completed forms, it will be necessary to complete or submit the following:
    • Photocopy of a government issued ID with a picture (driver’s license, passport, etc.)
    • Photocopy of your driver’s license (if you plan to volunteer as a driver for your Local Program)
    • Photocopy of your car insurance (if you plan to volunteer as a driver for your Local Program)
