DREAM Forms

Forms are annoying; there are no two ways about it. But they are important for the safety of the program and necessary for the fulfillment of our screening obligations to our insurance carrier. So get them done this once, and they are done forever! Hurrah! Click here for mentor forms, and click here for child forms below. New Mentor Forms:

In order to be part of the DREAM program, and covered by DREAM's volunteer insurance, all potential mentors must print out, read through, and keep on hand the following document:

• Mentor Boundaries, Driving Rules, and Child Abuse Information & Reporting Procedures This comprehensive packet covers the rules of conduct that help ensure the safety of everyone in DREAM, safe driving policies, and the signs and symptoms of child abuse as well as ways to deal with suspected abuse

• New Mentor Form Allows your picture to be used in DREAM media (stuff like newsletters, the website, collages, and photo albums), shows our insurance carrier that all of our mentors have read and understand DREAM's safety guidelines, and records driver information.
• Mentor Background Statement Please fill out this form accurately and honestly to provide DREAM with information on your background
• A statewide criminal background search:
  • VT Criminal Check Fill this out if you live in Vermont
  • NH Criminal Check Fill this out is you live in New Hampshire)
• Please get the following documents to the office:
  • Photocopy of Your Driver's License
  • Photocopy of Your Vehicle's Insurance

• New Child Form Provides basic contact information and parental concerns, as well as provides vital information for the safety of the child during DREAM time
