DREAM Forms

Forms are amazingly FUN; there are no two ways about it. They are also amazingly important for the safety of the program and necessary for the fulfillment of our screening procedures. So get them done this once, and they are done forever! Hurrah! Use the links below to print out your own forms.


Vermont DREAM Forms/Documents:

All mentors are required to participate in DREAM’s Mentor Boundaries, Transportation Guidelines, and Child Abuse Information and Reporting Procedures training with a Co-Chair or a Program Empowerment Director(PED). This comprehensive packet covers the rules of conduct that help ensure the safety and privacy of all of DREAM’s participants as well as safe transportation practices and how to identify and report signs and symptoms of child abuse.

FORMS
VT Mentors

Mentors must complete the following forms and give them to one of their DREAM Co-Chairs, their Local Program's PED or mail them to the DREAM office that supports your Local Program:

  • New Mentor Form includes:
    • Your contact information
    • Criminal Background and Sex Offender Registry check permission
    • Media Release
    • Parental Contact permission
    • A Mentor Background Statement
      • Driving History
      • Provide a copy your drivers license and vehicles insurance card
      • Criminal Background
      • Personal & Professional References
  • Mentor Pledge shows that mentors understand the commitment they have made to DREAM and shows our insurance carrier that all of our mentors have read and understand DREAM's Mentor Boundaries, Driving Guidelines, and Child Abuse Information & Reporting Procedures. This form is only to be completed after a mentor has completed DREAM’s Mentor Boundaries, Transportation Guidelines, and Child Abuse Information & Reporting Procedures training with a Co-Chair or a PED.

Other forms

    • All mentors from Dartmouth must complete a New Hampshire Criminal Background Check form.
    • If you are going to be a driver for DREAM activities using a personal vehicle or a vehicle provided by your college or university, DREAM needs a photocopy of the following:
      • Your Driver's License
      • Your Vehicle's Insurance

VT Children

Parents/Guardians of children must complete the following forms and return them to one of their DREAM Co-Chairs or mentors, a DREAM Office member or mail them to the DREAM office that supports your Local Program :

  • New Child Form includes basic contact and health information of child, consent to treat release, media release, and general release for participation in The DREAM Program, Inc. Completion of these forms is vital, for DREAM to create a safe, fun, and inclusive environment!
  • In addition, some Housing Authorities require that all new DREAM children have a Release Waiver completed:


Boston DREAM Forms/Documents

Greater Boston Area Mentors

In order to be part of The DREAM Program, Inc. and be covered by DREAM's insurance, all potential participants (mentors, children and volunteers) must complete the appropriate forms.

Mentors must complete the following forms and give them to one of their DREAM Co-Chairs, their Local Program's PED or mail them to the DREAM office at 41 Dearborn St Roxbury, MA 02119.

  • New Mentor Form includes:
    • Your contact information
    • Criminal Background and Sex Offender Registry check permission
    • Media Release
    • Parental Contact permission
    • A Mentor Background Statement
      • Driving History
      • Provide a copy your drivers license and vehicles insurance card
      • Personal & Professional References

Along with this from please provide a photo copy of a government issued ID (driver’s license, passport, etc…)

  • SafetyNET Check Form includes :
    • Your basic information
    • Criminal Background
    • A release for a FBI background check

  • Other forms
